Every artist tattooing at Tattoo Fest Green Bay must fill out this information. If you do not have a Wisconsin Tattoo Permit you can obtain it here. WISCONSIN ONLINE PERMITTING. Once permit is obtained from Wisconsin’s online portal, please download it and upload to the form below. You will also be uploading a copy of your government ID. Do not fill out this form until you have completed your licensing process.
You should bring a printed copy of your Bloodborne Pathogens Certificate to the festival with you. If you need a new BBP Certificate, we recommend this one, which is OSHA certified.
